Zelvimed is a telehealth platform operated by QuietScale LLC ("we," "us," "our") that connects patients with licensed healthcare providers for clinically-guided weight loss treatment, including GLP-1 receptor agonist therapy and related services. These Terms of Service ("Terms") govern your use of our website at zelvimed.com and all related services.
2. Eligibility
To use Zelvimed services, you must:
- Be at least 18 years of age
- Be a resident of a U.S. state where our services are available
- Provide truthful and accurate information during enrollment and medical intake
- Have the legal capacity to enter into a binding agreement
3. Nature of Services
Telehealth Platform
Zelvimed is a telehealth platform that facilitates connections between patients and licensed healthcare providers. We are not a medical practice, pharmacy, or healthcare provider. Medical decisions, including whether to prescribe medication, are made solely by the licensed physicians in our network based on their independent clinical judgment.
No Guarantee of Prescription
Completing an eligibility quiz, intake form, or payment does not guarantee that you will receive a prescription. A licensed physician will review your medical information and determine whether treatment is appropriate for you. If a physician determines that GLP-1 or other medication is not medically appropriate, you will receive a full refund.
Not Emergency Services
Zelvimed is not an emergency medical service. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.
4. Subscription and Pricing
Monthly Membership
Zelvimed operates on a monthly subscription model. Your membership includes a telehealth physician consultation, ongoing clinical monitoring, and medication (if prescribed) delivered to your door. Current pricing is displayed on our website at the time of enrollment.
Billing
Your subscription will automatically renew each month on the anniversary of your enrollment date. Payments are processed securely through Stripe. You authorize us to charge your payment method on file for each renewal period.
Price Changes
We reserve the right to adjust pricing with at least 30 days' advance notice. Any price changes will take effect at the start of your next billing cycle after the notice period.
5. Cancellation
You may cancel your subscription at any time by contacting us at [email protected]. Cancellation will take effect at the end of your current billing period. 6. Your Responsibilities
By using our services, you agree to:
- Provide accurate information: All medical history, personal information, and health details you provide must be truthful and complete. Inaccurate information could affect the safety of your treatment.

- Report side effects: Promptly notify your Zelvimed physician and/or seek medical attention if you experience adverse reactions to any prescribed medication.
- Personal use only: Medications prescribed through Zelvimed are for your personal use only. You may not share, sell, or transfer your medication to anyone else.
- Maintain account security: Keep your login credentials confidential and notify us immediately of any unauthorized account access.
7. Intellectual Property
All content on the Zelvimed website — including text, graphics, logos, design elements, and software — is the property of QuietScale LLC or its licensors and is protected by copyright, trademark, and other intellectual property laws. You may not copy, reproduce, distribute, or create derivative works from our content without prior written permission.
